Christian Community Health Center (CCHC), established in 1991, is a Federally Qualified Health Center and 501(c)(3) non-profit organization dedicated to providing high-quality primary care, oral care, and behavioral health services to underserved communities in Chicago. With over 27 years of service, CCHC has expanded its reach to include multiple clinic locations, ensuring accessible healthcare for all.
CCHC offers a comprehensive range of services to address the diverse needs of its patients. Primary care services encompass family practice and internal medicine, catering to individuals of all ages. Children’s health services support patients from prenatal stages through adolescence, while women’s health services provide a full spectrum of care for women at every life stage. In partnership with Sonrisa Family Dental, CCHC delivers dental care, including general and pediatric dentistry, crowns, fillings, orthodontics, root canals, implants, and urgent care. These dental services are available at the Chicago clinic located at 9718 S. Halsted Street.
Recognizing the importance of mental wellness, CCHC provides a wide array of behavioral health services across all clinic sites. Licensed clinicians assist with issues such as depression, anxiety, post-traumatic stress disorder, and substance use disorders. Services include psychosocial assessments, individual and group therapy sessions, and psychiatric referrals tailored to each client’s needs. As a Division of Substance Use Prevention and Recovery licensed provider, CCHC offers intervention and treatment for substance abuse, featuring certified alcohol and drug abuse counselors who conduct group-level interventions.
CCHC’s commitment to holistic care extends beyond medical services. The organization offers supportive housing programs designed to provide stability and case management to chronically homeless individuals and families, including those affected by domestic violence, HIV/AIDS, mental illness, and other chronic conditions. Programs such as Amani House, Footprints, EnHarmony, and Brainerd aim to promote self-sufficiency and improve overall quality of life for participants.
To ensure comprehensive patient support, CCHC operates an on-site pharmacy at the Chicago clinic on 97th and Halsted Street. This facility allows for convenient access to medications, with pharmacists collaborating closely with medical staff to assist patients in navigating prescriptions, insurance processes, and billing information. The pharmacy participates in the 340B program, enabling the provision of discounted medications to eligible patients.
