Transformations Wellness Center is a nonprofit substance use disorder treatment organization based in Klamath Falls, Oregon. The center serves individuals in Klamath County and other areas of Oregon who are seeking support for alcohol or substance use disorders.
The organization began operations in 2007 as a five-bed men’s residential treatment facility and later expanded into a larger co-ed residential program with additional outpatient services. Transformations Wellness Center operates residential treatment, outpatient treatment, substance use disorder assessments, counseling, and recovery housing for men and women. The program also offers peer support services and case management as part of its service structure.
Programming includes group and individual counseling and behavioral therapies such as cognitive behavioral therapy and dialectical behavioral therapy. The organization also provides life skills development, relapse prevention education, and support services related to recovery planning. Transformations Wellness Center works with community partners and accepts referrals from across the state of Oregon.
